First Choice Care offers Housing Stabilization Services, a Medical Assistance benefit tailored to individuals with disabilities, seniors, and those at risk of homelessness, aiding in the procurement and maintenance of housing. This service, part of the state plan Home and Community-Based Services (HCBS) comprises three components: Housing Consultation, Housing Transition, and Housing Sustaining.

Housing Consultation involves facilitating informed choices regarding housing transitions or sustainability and linking individuals with non-housing resources identified in their personalized plan. Housing Transition supports individuals in securing housing, while Housing Sustaining assists in maintaining it.

Eligibility
Eligibility criteria include having health insurance, being 18 years or older, having a documented disability or disabling condition, and experiencing housing instability. First, a Housing Consultant Specialist will collaborate with you to develop a housing-focused person-centered plan, ensuring tailored support to find and retain suitable housing.
